Nomination Criteria

Nominee:

Is an individual nurse, nurse leader or nurse-led team.
Demonstrates ability to address social needs.
Advocates and provides care for patients and families who are disadvantaged demographically, psycho-socially or economically.
Integrates social care into overall care.
Demonstrates strong communication skills.
Serves as a role model for their contributions to improve health equity and providing compassionate, quality health care.
